Job Description

Position: Senior Care Coordinator

Responsible to: Care Manager

Purpose of position

To be responsible for the efficient and timely delivery of care schedules for the care workforce ensuring the requirements of the Service Users are met.

Effective management, coaching and development of the care coordinator/s.

Liaising with external agencies such as hospital discharge teams, social services and brokerage teams for capacity and programming of new care packages. Ensuring the efficient programming and input of care packages inline with the operational standards and requirements of the business.

As and when required deliver care and promote a caring environment for Service Users through high standards of professional practice, which are conducive to the physical, emotional, social, intellectual and spiritual needs of the Service Users.

To ensure that each Service User receives care appropriate to their individual needs.

To implement the requirements of the CQC’s Essential Standards of Quality and Safety.

 

 

Principal Responsibilities

Coordination and management of the care schedules.

Daily monitoring of the care visits.

Programming of new care packages.

Liaise with external services such as Hospital Discharge Teams, Social Services, Brokerage Teams, District Nurses, Services Users and their families and/or representatives.

Ensure systems are correctly updated in an efficient and timely manner to reflect changes such as sickness, cancellation of visits, amendments to carers to visits.

Manage and develop the care coordinator/s. Conduct 8 weekly supervisions with care coordinators, ensuring all training needs are identified and met.

To provide out-of-hours service as contracted for the needs of the business.

To carry out general office duties, answer telephone, in line with your job requirements.

To assist in the investigation of complaints. Take appropriate action and report to the Care Manager, if required.

To ensure that the emotional, spiritual, physical, medical and material needs of the Service User are recognised, assessed and met

To provide personal care to Service Users to met the requirements of the business.

To assist the Care Manager in ensuring that there is good communication between and within staff at all levels, and to attend staff meetings, supervisions and appraisal interviews.

To undertake training to meet statutory and development needs of the position.

 

 

Person Specification

 

A suitable qualification in Care NVQ 3 (leading to NVQ 4 or above)

Car owner driver

Fully conversant in CM2000

Able to work well under pressure

Minimum 2 years experience in a similar role.

Self motivated

Excellent organisational skills

Knowledge of the local areas to include; Sandwell, Dudley, Walsall boroughs.

Excellent time management skills

Flexible

Caring

Sensitive to the needs of others

Excellent written and verbal communication skills

Committed to achieving high standards

Committed to training

Hours of work

48 hours per week on a shift pattern basis working 5 days over 7 between the hours of 6am to 11pm.

Out of Hours working required contractually as per the business needs.
